The Information We Collect and Use
By information we mean information about you collected through our Service website(s), including your email address and other information you provide to us by registering for our services or making requests for information about our services. Any information provided to us will be retained and used solely for the purposes of fulfilling your request for information, fulfilling a request through a Gabbert Clinic feature, performing and carrying out the terms of the Service, or communicating with you as a member of the Service.
Registration
Registration for the Service is not required to simply view the Gabbert Clinic. If you elect to register for the Service, Gabbert Clinic Vancouver, WA asks you for information that enables us to provide the Service. You will be registering with Gabbert Clinic Vancouver, WA on the form provided and such registration may require you to provide contact information such as your name and email address.

Clear Gifs (Web Beacons / Web Bugs)

We may use clear gifs in our HTML-based emails to let us know which emails have been opened by recipients. Profile

Gabbert Clinic is committed to complying with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations. We maintain appropriate administrative, technical, and physical safeguards to protect the privacy and security of protected health information (PHI) in accordance with HIPAA. Our employees are trained on HIPAA requirements and we have implemented policies and procedures to ensure compliance.

This notice provides information about how we may use or disclose protected health information. The HIPAA (Health Insurance Portability and Accountability Act of 1996) law allows for the use of the information for treatment, payment, or healthcare operations. By signing this form, you consent to our collection, use, and disclosure to other entities related to your case of your protected healthcare. You have the right to revoke this consent in writing, signed by you. However, such a revocation will not be retroactive. Consumer information is not shared with third-parties for marketing purposes.
I understand that my health information is private and confidential. I understand that Gabbert Clinic works very hard to protect my privacy and preserve confidentiality of my personal health information. The practice may call or text my phone number or other designated location (such as caregiver phone number) in reference to appointment reminders, home exercise programs, insurance issues and any matter pertaining to my clinical care, etc. I consent to receive SMS text messages from Gabbert Clinic – Hand Therapy and Occupational Therapy for appointment reminders, marketing messages, and general two-way communication. Message frequency varies. Msg&data rates may apply. Reply HELP for support. Reply STOP to opt out. I authorize Gabbert Clinic to leave voicemails regarding scheduling, insurance benefits and billing. The practice may mail to my home/email or other designated location any items that assist in carrying out my treatment such as appointment reminder cards and patient statements. I understand I can opt out of any type of communication at any time. See the remainder of our privacy policy for more information.
I grant permission to Gabbert Clinic to utilize artificial intelligence (AI) ambient listening technology during my session to better facilitate note taking and documentation of the session. This recording will be protected in the same manner as all other PHI. I understand that I have the right to decline the use of AI recording during my session.
I understand that signing this document means that Gabbert Clinic PLLC may request, use, and disclose my personal health information to help provide health care to me or my child, to handle billing and payment, and to take care of other health care operations. This may involve requests and disclosures to other healthcare providers, insurance companies, claim adjusters, vocational rehab workers, and other relevant, involved parties. Failure to sign may result in Gabbert Clinic PLLC declining to treat me.
